If you have the designer's contact information, could you pass it on to me? I discovered the same module and diagrammed it without realising it had already been done by someone else, and I would like to ask if she minds my diagrams being online.

Simple module, but...

I don't know an online source for this module. Diagrams should be in "Origami Tanteidan Convention No.13", but it might be hard to get access to that.
I do have a video on how to make this model, but have unfortunately not heard back from Miyuki Kawamura on whether she's ok with me putting it online. If she does in the future, I'd be happy to post it.
